Health History

In accordance with Colorado State Licensing, we require a variety of medical forms after registration. You can find a guide on how to complete these forms in the Parent Handbook. Your child will not be able to participate in Y programs without the required paperwork. 

A physical exam is required for your child to attend camp. The exam must be current within 12 months of the last day your child will attend camp. This Health History form must be completed and signed by a licensed physician. Form must be submitted at least two weeks prior to your child’s session. 

An immunization record within the past 12 months is required on an Approved Colorado Certificate. We do not require records of COVID-19 vaccinations. If your camper has an immunization exemption, please ensure that it is signed by a physician within the last 12 months on this form. For more information on these forms, please visit this Colorado Department of Public Health & Environment here. 

You must complete a state licensed asthma and/or allergy health care plan form and the YMCA’s medical authorization form for any condition requiring medication or emergency treatment. Forms must be submitted at least two weeks prior to your child’s session.

Bicycle Inspection

All campers who are bringing their own bikes to camp, must have a bicycle inspection form completed by a bicycle mechanic.

Financial Assistance

The Y believes all children deserve an overnight camp experience. That's why we provide scholarships to hundreds of campers each summer. For information on YMCA Camp Tumbleson Lake's financial assistance program, email our camp director.

Camp Store

We have t-shirts, sweatshirts, bracelets, stickers and more, ranging in price from $3 to $50. You can put money in your camper’s account before they arrive, so they can buy goodies throughout the week.
